We feel it most appropriate to have this survey completed separately
for the following age groups, as children have a continually changing
perspective as they grow.
- 5 to 8 yrs.
- 9 to 12 yrs.
- 13 to 18 yrs.
If you have children in more than one of these age groups please complete and submit as many of the surveys as is appropriate for your situation.
